EXAMINATION BY CITY COUNCIL LIBRARY STUDY COMMITTEE
1. All consultants=
(including architectural or engineering) reports as to repairs or renovations
needed at Forest Park, Liberty and East Springfield branches.
2. Copies of IRS Form 990 for fiscal years
ending June 30, 1993, 1994 and 1995.
3. Copies of financial statements of SLMA for
the years ending June 30, 1983, 1984, 1985, 1986, 1987, 1988, 1989, 1990, 1991,
1992, 1993, 1994 and 1995.
4. Stephen Kaplan of Kalicka Co., in a letter to
the editor on March 23, 2003, stated that in the last 7 fiscal years, SLMA
spent $17,427, 756 on capital expenditures and $3,799, 204 on library book
purchases. With respect to the capital
expenditures, describe each such expenditure, the year in which it was made,
and its total cost. As to library book
purchases, give the total amount spent on such purchases in each year of the
seven year period.
5. List all funds received from the City of
Springfield which were derived from bond issues of the City beginning January
1, 1987. In addition, enumerate every
project or improvement to the properties or holdings of SLMA with City bond
funds during said period along with the year of the grant or expenditure and
the specific amount of money spent on each said project.
6. A copy of the lease for the property on
Island Pond Road occupied by the East Forest Park branch.
7. All records indicating the cost and date of
the expansion of the Sixteen Acres branch, along with identification of the
source of the funds spent for that expansion.
8. A detailed list of all capital expenditures
with the cost and date thereof since January 1, 1985 in the following branches:
Forest Park
Liberty
East
Springfield
Mason
Square
Pine
Point
9. All documents which spell out the cost of the
recent renovation of the Indian Orchard branch and the source or sources of
funds used to pay the cost of the renovation.
10. Documents which show the cost of all capital
improvements to the Central Library since January 1, 1985. Along with this, please list the source or
sources of funds for any and every capital improvement to said facility during
that period of time.
11. Copies of the AMayor=s Recommended Budget@ for Library and Museum operations for
the last ten years.
12. Detailed listing and breakdown of grants or
revenues from the Commonwealth of Massachusetts for each one of the last five
fiscal years.
13. Detailed listing and breakdown of grants or
revenues from the federal government for each one of the last five fiscal
years.
14. Copies of any collective bargaining contracts
or letters of understanding as to job rules with some or all of the library
employees.
15. Tables of organization for library employees
for the entire system as of July 1, 2000, 2001, and 2002.

For each one of the last five fiscal years, list the cost of fringe benefits
for retirement to

all library employees, b) all museum employees, and c) to all other SLMA
employees; give the same five year breakdown for the same three groupings of
employees for 1) health insurance, 2)workmen=s
compensation, 3) unemployment insurance and 4) contributions (e.g. mayor=s budget item #2 0100
610 5176).
20. Records for the last three fiscal years
setting forth the total number of volunteers working in the libraries with the
cumulative total of their hours of volunteer service.
21. Records for the last four fiscal years
showing the total revenues derived annually by the SLMA from the Springfield
School Department.
22. Copies of any contracts, letter agreements or
understandings between SLMA and the School Department in each of the last four
fiscal years relative to the operation, staffing and operating the Brightwood
branch library. In addition, set forth
the total amount of revenue derived by the SLMA in each year for the operation
of the Brightwood branch.
23. A complete list as of July 1, 2002 of each
and every perpetual trust which is categorized as Apermanently
restricted@ with the
value as of said date of each trust which has been set forth along with the
information as to which trusts were designated for library purposes, which for
museum purposes and which for no specific library or museum purpose.
24. The June 30, 2002 SLMA balance sheet shows
the unrestricted investments as having a value of $3,972,232. Give a detailed exposition of said
investments, setting forth the value of each and the designated purpose of
their use, if any.
25. The June 30, 2002 SLMA operating statement
shows trust fund income of $1,466,231 for the year. Delineate the amount of income produced from
each investment and spell out in each case whether the investment had been
designated by each respective donor as being for the libraries of the museums or
undesignated, as the case may be.
26. Copies of each and every report made by the
SLMA to the Massachusetts Board of Library Commissioners in the last five full
fiscal years relative to the operations of any or all of the libraries operated
by SLMA in Springfield.
27. For each of the last five full fiscal years,
set forth the circulation and visitors per branch (include individual figures
for each of the nine branches and also the Central Library). Furthermore, in every instance, set forth the
number of hours per week that each facility was open.
28. For each of the last five full fiscal years,
set forth the number of visitors to each one of the museums operated by
SLMA. In every instance set forth the
number of hours per week that each facility was open.
29. Set forth in individual detail all funds
raised by SLMA=s
development and marketing activities in each of the last five fiscal
years. (List only contributions that
exceed $100, but you may aggregate the totals raised in each year of gifts of
$100 or less.)
30. Set forth in detail any and all costs of SLMA=s development and marketing activities
in each of the last five fiscal years.
Include in this response an identification of all employee positions
involved during those years in the Association=s
development efforts.
